Connect Care

Terms of Use

Important: not HIPAA-compliant

Connect Care is not currently HIPAA-compliant. The platform and its hosting provider do not hold a Business Associate Agreement, so the legal safeguards required for protected health information are not in place.

Please do not submit Protected Health Information (PHI) — diagnoses, medication details, clinical notes, test results, insurance information, or photos that identify a resident — until the facility confirms that a suitable agreement is in place. Use phone calls, the resident's medical record, or direct conversation with staff for anything clinical.

Access controls, audit logging, and automatic sign-out are still in force as privacy safeguards. See the Notice of Privacy Practices.

Who may use Connect Care

Accounts are created by invitation only, by the facility operating this service. Accounts are personal: you may not share credentials or let another person post under your name. Family accounts are limited to residents linked to them by the facility.

Acceptable use

  • Do not submit Protected Health Information while this platform is not HIPAA-compliant (see the notice above).
  • Post only accurate, respectful information about care.
  • Do not share information about a resident other than your own loved one or assigned resident.
  • Do not export, screenshot, or forward Protected Health Information outside authorized channels.
  • Connect Care is not for emergencies. Call 911 or facility staff directly for urgent needs.
  • Connect Care does not replace the resident's clinical record or medical advice.

Content and ownership

Care updates, photos, notes, and clinical entries belong to the facility and the resident's record. Families grant the facility permission to store notes and photos they upload. The facility may remove content that violates these terms or its privacy policies.

Availability

The service is provided as-is, without guarantee of uninterrupted availability. Planned changes are published to the facility's administrators through release notes.

Ending access

The facility may deactivate an account at any time — for example when a caregiver leaves or a resident is discharged. Historical records remain in the resident's file according to the retention schedule in the Notice of Privacy Practices.

Questions about these terms should go to the facility administrator.
